Concierge Service

We offer a flat fee for service practice to allow for more time to deliver through care via monthly membership or fee per visit. Members have more time and access to our providers and services.

Your insurance is used for all other services such as prescriptions, testing, labs, specialty referrals or equipment.

Can I get a same-day appointment?

We offer same day appointments in office, home or virtual contact.

What health insurance do you accept?

We now accept select health insurance plans including Aetna, United Healthcare, Excellus BC/BS, Empire BC/BS, Anthem, Fidelis, Medicare and Medicaid
As a reminder, if you use your insurance for primary care, your insurance limits the level of time/care we can spend with you per visit. That is why we urge membership to our practice.

School Physicals

We offer appointments quickly for school needs or employment needs.

Consultations

Need to talk about medical issues that may have consequences in your life personally or legally. Or do you need a professional medical opinion on a matter.

How far will we travel for house calls

The first 15 miles of travel are free, after that, there is a fee of 0.60 cents per mile.

Cost for services/fee schedule

Flat fee costs example:

annual physical $60-$80

sports/school physical-$50

consultations- $80 per hr

urgent care in office- $50 per half hr

urgent care house call- $60 per half hr

medical marijuana cert. -$100

prescription antibiotics- $5-20

membership- $100 per month

strep test- $20

urinalysis- $20

hemoglobin A1C result in 5 min. - $25

Can I be seen as needed without membership

You can be seen without a membership and pay the flat fee for service rendered or use your health insurance if we accept your health plan.
